Direct Bilirubin — What It Means in Your Blood Test

New to reading lab reports? Start with our guide to reading a blood test report

Direct bilirubin is the form your liver has already prepared to leave the body in bile.

Direct Bilirubin zooms in on just the pigment that is ready to leave your body. Looking at this readied portion on its own can point toward where a hold-up might be happening.

Also known as: Conjugated Bilirubin, D. Bili, Direct Bili, DBil

Kept in balance

This isn't a case where higher or lower is better. Your body keeps it in a narrow band, and both directions can matter.

What is Direct Bilirubin?

This is the water-soluble form (conjugated) that the liver has chemically changed by attaching a sugar group. Once altered, it can be excreted through the bile ducts into the gut.

Why is Direct Bilirubin tested?

You have this test to help show whether raised bilirubin comes from your liver or from your bile ducts.

What is a normal Direct Bilirubin range?

India
No India-specific national reference interval is published for this parameter; your lab's printed range is the most reliable comparison.
Global
0.0–0.3 mg/dL

Ranges vary by lab, method and equipment — the range printed on your own report is the one that applies to your result.

What does your Direct Bilirubin value mean?

There is no widely-accepted set of value bands for this parameter. Compare your result with the reference range printed on your own report, and discuss it with your doctor.

What does a high Direct Bilirubin result mean?

A raised level can mean bile is not flowing well out of your liver or bile ducts. It is worth discussing with your doctor.

What does a low Direct Bilirubin result mean?

A low level is normal and usually not a concern.

What can affect your Direct Bilirubin result?

  • Bile flow
  • Liver cell function
  • Some medicines
  • Fasting before the test
  • Blood cells breaking in the sample
