What Free PSA cannot tell you

Free PSA on its own tells you very little, and it is never a test for cancer. Its value is only in the ratio to total PSA, which a doctor reads with everything else. A number here does not confirm or rule out anything by itself [AUA, 2023].

What does Free PSA do in your body?

Free PSA looks at the portion of the prostate protein that is not bound to other proteins. Paired with total PSA it gives a ratio that can help a doctor judge, in the borderline range, whether to look further [AUA, 2023].

Free PSA is the fraction of prostate-specific antigen circulating without being attached to blood proteins. Laboratories measure it so it can be compared against the total amount.

What can raise Free PSA

Common, non-cancer causes

What does your Free PSA value mean?

There is no widely-accepted set of value bands for this parameter. Compare your result with the reference range printed on your own report, and discuss it with your doctor.

What does a high Free PSA result mean?

A higher free fraction, giving a higher free-to-total ratio, is generally the more reassuring direction. Your doctor reads it together with your total PSA and history.

What does a low Free PSA result mean?

A lower free fraction lowers the ratio, which a doctor may look into further. On its own it decides nothing, so it is read with the total PSA and other tests.
