What Does a High ESR Mean?

A high erythrocyte sedimentation rate (ESR) indicates that red blood cells settle faster than usual in a test tube [Brigden, 1999]. This pattern often reflects active inflammation or protein changes in the body [Brigden, 1999].

What does the ESR test measure?

The erythrocyte sedimentation rate measures how quickly red blood cells settle to the bottom of a tube in one hour [CLSI, 2011]. Normally, red blood cells settle relatively slowly because of their negative electrical charge [Brigden, 1999].

The result is reported in millimetres per hour (mm/hr) [CLSI, 2011]. It provides a broad index of physical changes in the blood plasma [Brigden, 1999].

Why does ESR rise during inflammation?

Inflammation stimulates the liver to produce proteins such as fibrinogen and immunoglobulins [Gabay and Kushner, 1999]. These positively charged proteins bind to the surface of red blood cells [Brigden, 1999].

This binding reduces repulsive forces between the cells, causing them to form stacks called rouleaux [Brigden, 1999]. Heavier cell stacks fall through the plasma much faster, resulting in a higher ESR reading [Gabay and Kushner, 1999].

How does ESR differ from CRP?

C-reactive protein (CRP) is a specific protein that increases rapidly in response to acute inflammation [Gabay and Kushner, 1999]. CRP levels can rise within hours and decline rapidly as inflammation subsides [Markanday, 2015].

In contrast, ESR rises more slowly and can remain elevated for weeks after inflammation resolves [Markanday, 2015]. ESR reflects changes in multiple plasma proteins and cell shape, whereas CRP measures a single protein [Markanday, 2015].

What ordinary factors can cause a high ESR?

ESR values naturally increase with advancing age [Brigden, 1999]. People assigned female at birth also generally show slightly higher baseline ESR levels than males [CLSI, 2011].

Pregnancy significantly increases fibrinogen, which naturally raises ESR readings from the second trimester onward [van den Broek and Letsky, 2000]. Mild temporary viral infections, anemia, or technical lab variations can also increase the rate [Brigden, 1999].

Why is ESR considered a non-specific test?

ESR is described as non-specific because an elevated value does not pinpoint a particular disease or organ [Brigden, 1999]. It signals the presence of an altered protein state, but not its underlying cause [Markanday, 2015].

Clinicians typically interpret an ESR value alongside a physical examination, medical history, and other laboratory tests [Brigden, 1999]. A normal ESR also does not completely rule out an active medical condition [Brigden, 1999].
